Dr. Carolyn Gosse brings over two decades of expertise in integrated care, healthcare administration, and pharmacy to her roles. Currently serving as Clinical Vice President at University Health Network (UHN), Dr. Gosse is leading the development of UHN@Home, a pioneering initiative providing integrated care for thousands of UHN patients. As executive lead for the older adult strategy, she is co-leading the development of a UHN-wide approach to the care of older adults in an integrated system.
Her leadership extends to St. Joseph’s Home Care, where she served as President, overseeing strategic planning and operational excellence. Under her guidance, St. Joseph’s Home Care launched innovative community models of care, such as COVIDCare@Home. Dr. Gosse’s commitment to advancing integrated care is evident through her role as Vice President Integrated Care at St. Joseph’s Health System, where she supported the development of sustainable care models across the continuum.
Dr. Gosse, who is also a Doctor of Pharmacy graduate from the University of Florida, is a recognized leader in healthcare innovation and patient-centered care. She is supported by extensive professional development.
